Western Victoria PHN (WVPHN) receives federal and state government funding for commissioning projects and services to address priority primary health care needs in our region and to improve coordination of care.
How commissioning works
Our commissioning is strategic and takes an evidence-based approach to planning and purchasing health services.
Through commissioning, WVPHN focuses on ensuring health services provide patient-centred outcomes, with an emphasis on the health priority target areas of:
- Mental health
- Aboriginal and Torres Strait Islander health
- Population health
- Health workforce
- Digital health
- Aged care
- Alcohol and other drugs treatment.
We work with communities and health care providers throughout our region to develop, deliver and monitor models of care which put patients first to improve the health of our wider community.
