Western Victoria Primary Health Network (PHN) commenced operations on 1 July 2015. Continuity of patient services is a key focus in 2015/16. This new, not for profit organisation is responsible for delivering on the following two objectives set by the Federal Government;

  1. Increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes.
  2. Improving coordination of care to ensure patients receive the right care, in the right place, at the right time.

We have engaged staff, stakeholders and the Western Victorian community to develop a new strategic plan for the Western Victoria PHN. The summary document our 2016/17 Strategic Intent Statement is now available.
