Western Victoria PHN (WVPHN) is pleased to invite organisations currently delivering or those working towards delivering after hours primary health care services to develop innovative initiatives that integrate and streamline after hours service provision across the western Victoria region.
The After Hours Research and Development Models aim to fund (up to $1.2 million across the whole WVPHN region) projects that address the findings and key themes from the after hours co-design process (completed November 2019 – February 2020) and objectives of the After Hours Primary Health Care program, with a particular focus on:
- Care coordination and workforce development
- Access to after hours primary health care services
- Aged Care
Projects funded under the After Hours Research and Development Models may address one or more of these priority areas and are also required to align with one or more of the after hours design constraints socialised and affirmed through the after hours co-design process.
