Participate in the GoShare Heart Care Pilot Project and receive $1,000 (ex GST) to use GoShare Voice AI calls to engage with patients who have been recently discharged from hospital after experiencing heart failure
Western Victoria Primary Health Network (WVPHN) is inviting general practices from across our region to submit an Expression of Interest to participate in the pilot project and receive funding as an opportunity to improve health outcomes for patients who experience heart failure.
The project aims to collect real-time health outcome measures and provide educational resources to promote self-management. This will be done by sending a series of structured artificial intelligence (AI) generated calls, in the form of GoShare Voice, and digital content bundles with general advice to the patient on looking after themself after heart failure.
Eligibility criteria
- Must be an accredited practice within the WVPHN catchment area
- Must be a user (or willing to be) of the GoShare platform
- Must nominate a practice staff member who has the capacity to complete required activities.

How will the project work?
GoShare Voice AI calls will be structured in two ways:
- By offering patients the opportunity to answer a Patient Reported Measures Outcomes (PROMs) survey, receiving educational resources as a result of the completion of the survey (there is also the option to complete an online survey as an alternative).
- Patients will also receive a call from GoShare Voice to ask questions relating to their condition. Responses will either be generated from peak body content such as the Heart Foundation, or for clinical questions the patient will be directed to contact the clinic directly.
