Primary health service providers, people with chronic conditions and their carers are invited to share their experience and knowledge of chronic conditions care services in western Victoria at one of a series of upcoming workshops.
Western Victoria PHN (WVPHN) wants to hear from our community as the first step in redesigning WVPHN-commissioned chronic conditions care services in our region to ensure they are provided to communities of greatest need.
We are seeking input from:
- Service providers including GPs, nurses and allied health professionals
- People diagnosed with one or more chronic conditions including but not limited to diabetes, asthma and heart disease along with their carers.
The online workshops form the initial stage of a co-design process to enable WVPHN to commission improved chronic conditions care services using new and innovative ideas for the future.
If you are unable to attend a workshop, other opportunities to contribute will be available via our online Meeting Place consultation platform.
Workshops for people with chronic conditions and carers will be held on Monday, 29 March and Tuesday, 30 March.
Workshops for primary health care providers will be held on Tuesday, 30 March and Thursday, 1 April.
Please note, a courtesy reimbursement payment will be available for eligible attendees.
