Western Victoria PHN (WVPHN) has announced South West Healthcare as the new lead provider for the Services and Treatment for Enduring and Persistent Mental Illness (STEPMI) program in the Great South Coast region.
STEPMI supports clients with severe and complex mental health illness by improving coordination of their clinical care. The program assists clients to access ongoing community-based supports and utilises evidence-based and individualised treatment plans.
South West Healthcare began service delivery in August 2021 and is currently accepting referrals for the STEPMI program.
Referrals into the STEPMI program by service providers must be supported by general practitioners (GPs), paediatricians or private psychiatrists. It is expected that mental health nurses will work with general practitioners and/or psychiatrists to provide coordinated clinical care for people with severe and complex mental illness. This care will be delivered in line with a GP Mental Health Treatment Plan or equivalent, developed by the GP or psychiatrist.
The preference is that referrals are made warmly directly through or alternatively via the Referral Point Primary Mental Health Care Referral Form July 2020 (PDF 5.1MB).
