Uniting (Vic Tas) Limited has been selected by Western Victoria PHN (WVPHN) as the new lead provider for the Services and Treatment for Enduring and Persistent Mental Illness (STEPMI) program in Wimmera Grampians.
The program assists clients with severe and complex mental health illness to access ongoing community-based supports and utilises evidence-based treatment to offer individualised treatment plans for clients.
The STEPMI program for Wimmera Grampians is a joint partnership between Uniting Ballarat and Wimmera. It is currently in its establishment phase and is expected to be fully operational in May 2021.
Referral into STEPMI
Referrals into the STEPMI program by service providers must be supported by General Practitioners (GPs), paediatricians or private psychiatrists. It is expected that mental health nurses will work with general practitioners and/or psychiatrists to provide coordinated clinical care for people with severe and complex mental illness. This care will be delivered in line with a GP Mental Health Treatment Plan or equivalent, developed by the GP or psychiatrist.
The preference is that referrals are made warmly directly through Uniting Vic Tas Limited or alternatively via the Referral Point Primary Mental Health Care Referral Form July 2020 (PDF 5.1MB).
