2017 Population Health Planning Needs Assessment

Western Victoria PHN is focussed on working with health service providers, users and communities to develop and improve access to the right primary health services, in the right place, at the right time. As part of this process, we have conducted our 2017 Population Health Planning Needs Assessment (Needs Assessment), collecting and collating information to identify systems and service development priorities, building on our previous work and knowledge. This will inform Western Victoria PHN's regional planning, commissioning and integration opportunities.
