Market Sounding Sessions for a New Model of Service Delivery for Chronic Conditions Service Providers – Ballarat Goldfields – 30 August

Western Victoria Primary Health Network (WVPHN) would like to get the perspective of chronic condition service providers as we move toward a place-based model for future services.

WVPHN is committed to ensuring the chronic conditions support services we commission are delivering the right outcomes for both patients and health providers.

Throughout 2021-2022, WVPHN took the first steps by consulting with health professionals and people with lived experience as part of a co-design process. From that process, a proposed enhanced service model has been developed.  We now want to work with service providers through market sounding sessions to understand the feasibility of the enhanced model. The marketing sounding will also provide an open and free space to discuss the enhanced service model and explore opportunities for partnership development.

Chronic condition support organisations and services – large and small – are invited to take part in the market sounding. This includes both current and non-current providers in the WVPHN region.
