Western Victoria Primary Health Network (PHN) has engaged Claire Edwards from b.school4change, as the consultant to lead the after hours sub-regional co-design project. This project has been initiated to assist services already delivering and those interested in delivering after hours primary health care to participate in a co-design process and (re)design collaborative, client centred and financially viable after hours service model(s) for the communities across western Victoria.
Through the provision of funding that will be made available via a procurement process in early 2020, Western Victoria PHN will support the design and implementation of localised after hours service models that are identified through the co-design process. These project/s and/or model/s will be required to be innovative and sustainable, address systemic after hours issues and improve the integration of after hours services.
Options to participate include both an on-line forum as well as co-design workshops. The consultation phase, which commenced in November 2020, will be used to understand current service provision, gather evidence of best practice examples and to determine the key requirements of best practice after hours service model(s).
Please find below a weblink to the after hours sub-regional co-design project on-line forum where all project information and on-line engagement will be conducted: https://phn.oursay.org/home
