Western Victoria Primary Health Network (WVPHN) partners with a variety of organisations to help improve community access to quality primary health care services and support.
Urgent and After Hours Care
WVPHN works with the Victorian State and Australian governments to help deliver urgent and after hours care options to support our communities and primary health care providers. These include Urgent Care Clinics.
Dementia Support
WVPHN has worked with Dementia Support Australia and other primary health networks to develop local Dementia Directories and information sheets to help people with dementia and their families better understand available support and services.

Care Finder
WVPHN has commissioned Care Finder service providers to deliver face-to-face support that helps people better navigate, understand and access aged care and other relevant services and supports. The care finder program is designed to complement My Aged Care by offering localised and face-to-face support, specific to the needs of older adults across western Victoria.

Community Transport
WVPHN is partnering with the Royal Flying Doctor Service to deliver a community-based patient transport service program for supporting people in rural and remote areas who are often unable to arrange transport for travelling to and from medical appointments. This state-wide program is also run in conjunction with Gippsland Primary Health Network and Murray Primary Health Network
In western Victoria, the service is currently available in Warracknabeal and St Arnaud.
