The Your Care Path program supports people with a chronic condition and who live in a rural or remote area. The program provides holistic support with set outcomes for their health and wellbeing..


Your Care Path offers the following benefits:

  • Health consumers will work toward a set of personal goals.
  • Programs will have flexibility to deliver services face-to-face as well as via telehealth.
  • There will be opportunities for support services to be delivered in group settings and/or on an individual basis.
  • Where needed, services can be tailored to be culturally appropriate.
  • Support will also be available for early intervention to help consumers at risk of developing one or more chronic conditions

Accessing Your Care Path

Your Care Path is available to anyone with at least one chronic condition or anyone who is medically determined by their primary treating clinician to be at risk of developing one or more chronic conditions within the next six months. You can contact the local service provider directly or be referred via your GP.


Covers the municipalities of Central Goldfields, Hepburn, Moorabool, Pyrenees.

Colac, Coast and Otways

Covers the municipalities of Colac Otway and parts of the Golden Plains and Surf Coast shires.
