Healthy Ageing Hubs provide older people with access to information, services and programs for living well and remaining socially connected within their community.
Through funding from the Australian Government’s Department of Health, Disability and Ageing, Western Victoria Primary Health Network (WVPHN) has a program to commission hubs across our region to support healthy ageing and reduce pressure on local health services.
The hubs are locally managed and deliver positive ageing activities and support tailored to meet the needs of their communities. Hubs are open to any residents in western Victoria aged 50 years and over and to First Nations residents aged 45 years and over.
Coming soon
Following a tender process in 2025, three new Healthy Ageing Hubs will commence in early 2026:
- AllCare Physiotherapy will run a hub from its clinic in Colac
- Grampians Community Health will operate two hubs: one at its offices in Horsham and the other will be a mobile service operating throughout the Shires of Hindmarsh, West Wimmera and Yarriambiack.
More details will be available in the new year.
Current Healthy Ageing Hubs in western Victoria
WVPHN provided funding in 2024 to establish the first round of following Healthy Ageing Hubs that are continuing to offer support and services to their local communities.
Clunes Neighbourhood House
| Based in Clunes, the Living and Ageing Well in Hepburn program also supports older people living in Creswick, Daylesford and Trentham.

| The mecwacare Health Ageing Hub supports communities in and around Ballan and Ballarat including local farming communities.
