Western Victoria Primary Health Network (PHN) is responsible for improving the efficiency, effectiveness and coordination of health services for patients, particularly those at risk of poor health outcomes. 

Collaborative working partnerships across the health system are key to ensure positive change.  

Western Victoria PHN has four regional centres based in Ballarat, Geelong, Horsham and Warrnambool.

We will establish four GP led Clinical Councils and four Community Advisory Committees for each regional centre. These Councils and Committees will provide advice directly to the Board on the specific health needs of each region.
