Advance care planning (ACP) is a process of discussion and shared planning for future health care, involving both the person (and family) and their health care professionals.

ACP focuses on the individual and provides each person the opportunity to develop and express their preferences for care. It includes their personal beliefs and values, and explores their understanding of their current and anticipated future health status and the treatment and care options available.

Victoria's advance care planning strategy, Advance care planning: have the conversation - a strategy for Victorian health services 2014 - 2018, aims to increase opportunities for people to develop advance care plans and for these to be activated.

Associate Professor Charlie Corke, Intensive Care Specialist at Barwon Health explains the importance of Advance Care Planning in this short video.

The SBS Insight program hosted an episode on ACP 'Do Not Resuscitate', featuring local GP, Dr Ric Milner, A/Prof Charlie Corke and local patient Monica Hayes.
