Advance care planning (ACP) is a process of discussion and shared planning for future health care, involving both the person (and family) and their health care professionals.
ACP focuses on the individual and provides each person the opportunity to develop and express their preferences for care. It includes their personal beliefs and values, and explores their understanding of their current and anticipated future health status and the treatment and care options available.
Victoria's advance care planning strategy, Advance care planning: have the conversation - a strategy for Victorian health services 2014 - 2018, aims to increase opportunities for people to develop advance care plans and for these to be activated.

Ballarat Health Services
Once an individual gives permission to share an advance care plan:
- Fax it to Ballarat Health Services Advance Care Planning Coordinator on 5320 6493
- Attach the patient's medical record
- Mark the document: Advance Care Plan for filing in Alerts
- Ensure the patient's full name, date of birth, address, BHS Ur number (if known) and a telephone contact number for the patient is included.
Western Victoria Primary Health Network is collaborating with Barwon Health to promote Advance Care Planning (ACP) awareness in general practice. The Advance Care Planning program at Barwon Health promotes an individual's understanding about their future treatment options and assists individuals to document their wishes and preferences in an Advance Care Plan.
