Part-Time hours: 32-38 hours per week

The Team Manager – Population Health will actively engage and participate with Central Highlands Rural Health Management and Community Health and Wellbeing colleagues to facilitate operational management of the Population Health team to effectively implement health promotion programs and initiatives within the health service and throughout the Hepburn Shire. The position leads the implementation of an evidence-based 2021-2024 Integrated Health Promotion Plan, which includes a focus on increasing healthy eating, active living and mental health and wellbeing.

Details of Appointment:

Permanent ongoing role offered at 64 hours up to 80 hours per fortnight.

Central Highlands Rural Health (CHRH) is a rural health organisation that operates five campuses across Hepburn Shire and the Macedon Ranges in the Central Highlands of Victoria in Australia, north and north-west of Melbourne. It was formed through the voluntary amalgamation of Hepburn Health Service and Kyneton District Health on the 30the November 2019. Each of our campuses retains the name of the township in which they are situated – Clunes Health, Creswick Health, Daylesford Health, Kyneton Health, and Trentham Health.
