Portland District Health & Heywood Rural Health are currently seeking an:
Experienced Aged Care Nurse Practitioner to work across Portland District Health Residential Aged Care (30 beds) and Heywood Rural Health Residential Aged Care (45 beds) Facilities.
The position will be responsible for operationalising our Aged Care Nurse Practitioner Residential In-Reach Collaborative Service.

This 12 month position is available immediately, for a suitable applicant.
Hours are negotiable split between Portland and Heywood Residential Aged Care Facilities.
