WVPHN has initiated Aged Care Pharmacist Connect (ACPC) to facilitate safe and sustainable partnerships between Residential Aged Care Homes (RACHs) and pharmacists to benefit residents as part of the Aged Care On-site Pharmacist program.
By participating in ACPC, pharmacists and RACHs will be helping deliver:
- Safer medicine useÂ
- Better communication with prescribers and familiesÂ
- Stronger governance and quality outcomesÂ
- Fewer avoidable hospital transfers.Â
How it works
| Step 1 – Apply | Complete and submit the application form below. |
| Step 2 – Review | WVPHN’s Aged Care Team will review applications to confirm eligibility, readiness (eg. eNRMC), and pharmacist credentialing. Â |
| Step 3 – Introduction | The team will then facilitate an introduction between RACHs and interested pharmacists or community pharmacies. |
| Step 4 – Support | WVPHN can provide templates, resources, and guidance to help embed the pharmacist role into clinical governance, QUM activities, and quality indicator reporting. Â |
