About GP Aged Care Match
GP Aged Care Match aims to provide mutual benefits for general practice and residential aged care homes while supporting key commitments to preventative health care for older people in our community.
Through the program, WVPHN wishes to ease concerns and barriers to offering primary care in RACHs by taking on the responsibility for identifying suitable matches between GPs and homes.

How it works
| Step 1 | Complete and submit the application form below. |
| Step 2 | WVPHN’s Aged Care Team will review each application by assessing the geographic location of applicants, capacity and aged care preferences. |
| Step 3 | An Aged Care Team member will contact applicants to discuss suitable options and, where appropriate, facilitate an introduction between GPs and RACHs to initiate a new partnership or to further build existing relationships. |
Benefits
As our population ages, the number of people living in aged care homes is likely to increase. Ensuring their access to quality and ongoing healthcare is essential.
Matching GPs with local RACHs will go a long way toward achieving this by creating new partnerships for care and wellbeing. In turn, the partnerships will deliver individual and shared benefits for both GPs and aged care home providers.
