All practitioners in the primary care sector have are key in delivering best practice chronic conditions management.
Coordinated and collaborative approaches to treating and supporting individuals living with one or more chronic diseases are recognised as offering the best outcomes.
Therefore, Western Victoria PHN and local health care providers are delivering programs and initiatives that focus on a patient’s needs at every point of their journey.
Chronic conditions management services redesign
As part of WVPHN’s commitment to supporting the delivery of patient-focused health services across our region, we are currently undertaking a far-reaching process to redesign services we commission including chronic conditions management services. We are working with our communities to develop new and innovative ideas to target health services to the people who most need them.
During July and August 2021, health providers and other interested members of the community have opportunity to contribute to the redesign of the chronic conditions management support programs commissioned by WVPHN.
