Your Care Path Chronic Conditions Support

Your Care Path is a new chronic conditions support program that will be available to people living in rural and remote areas of western Victoria. From January 2024, it has replaced the Chronic Conditions Model of Care programs in these areas.

 

What is Your Care Path?

Western Victoria Primary Health Network (WVPHN) has designed Your Care Path as part of a new regional care partnership approach to delivering primary health care services. This involves formal agreements among complementary health support agencies to ensure services to health care clients are coordinated and delivered to meet the individual’s needs.

These services are more collaborative, informative, supportive and culturally inclusive and better coordinate and promote recovery and personal wellbeing.

Your Care Path is closely aligned with Australia’s Primary Health Care 10-Year Plan and its emphasis on person-centred primary health care that is integrated, future-focused and locally delivered. Similar to the Chronic Conditions Model of Care program, Your Care Path will still only be available in areas classed as rural and remote under the Modified Monash Model (that is to say, areas classified MM3 and higher).

Types of services

The types of programs delivered to each client will depend on individual needs. These can include but are not limited to diabetes education; exercise programs; occupational therapy; physiotherapy; podiatry; respiratory services; social work; speech therapy; and weight management. Services may be delivered via individual and/or group consultations or therapy sessions by a qualified allied health clinician or nurse. The program also has flexibility to offer services face-to-face or via telehealth.

Goldfields

Covers the municipalities of Central Goldfields, Hepburn, Moorabool, Pyrenees.

Colac, Coast and Otways

Covers the municipalities of Colac Otway and parts of the Golden Plains and Surf Coast shires.
