Remote Patient Monitoring

Remote patient monitoring (RPM) is a telehealth tool that involves the use of digital health technology to provide real time links between health providers and their patients.

RPM helps remove barriers for patients who may currently face issues with attending regular appointments including people living in rural and remote areas and those with mobility difficulties. It helps empower patients and improves monitoring of health conditions in between appointments with health providers.

Digital technology used for remote monitoring of health conditions can include apps, services and mobile devices and tool that enables the delivery of real-time health readings to GPs and other practitioners. Benefits include early detection of potential health issues and reduced chance of hospitalisation.
