HealthPathways is the central source of information for GPs and other primary health care providers in our region. HealthPathways provides condition specific information on assessment, management and local referral options for clinician use during consultation. Western Victoria Primary Health Network has partnered with Barwon Health to facilitate HealthPathways development, implementation and evaluation.

In August 2013, the HealthPathways website was launched. By July 2016, there were 340 localised pages on the site, made up of 202 clinical pathways, the remainder are referral or resource pages. The HealthPathways website is dynamic with more pathways being localised every week and pathways are regularly updated and reviewed.
Clinical streams include orthopaedics, paediatrics, mental health, urology, general medicine, endocrinology, respiratory, colonoscopy, ENT, nephrology, cardiology, gynaecology and oncology.
HealthPathways aims to build primary health care skills and capacity, improve initial assessment and workup, encourage the creation of high quality referral information and ultimately reduce patient waiting times.
Pathways are designed to be quick and easy to use and access. They are relevant and targeted. Integrating local service information is a key element, providing details of local agencies and services that GPs can refer to. 'Best practice' management is the aim – and useful clinical reminders of the red flags and pitfalls in specific presentations are highlighted.
Another key goal is to promote closer relationships between specialists, GPs, allied health professionals and relevant community agencies - contributing towards a more connected health system, with both quality and efficiency gains for the patient. An essential element to the HealthPathways program is the establishment of clinical stream specific workgroups, enabling cultural change, by improving communication and collaboration between GPs and specialists.
