Supporting the Palliative Care Journey of Patients in General Practice – Online – 19 June

Please join this Victorian Primary Health Network collaborative event, for a general overview on the palliative care journey of patients accessing general practice, and how digital health platforms can support optimisation of care.

Presenters:

Dr Elise Davey is a GP and practice principal at Kensington Hill Medical Centre, Leopold Victoria, having joined in 2017 and specialised in General Practice. Dr Elise has a strong interest in geriatric care and is a proud provider of care to patients residing in residential aged care facilities. Dr Elise is also a current board member of Hospice Foundation Geelong.

Dr Paul Molina Chavez migrated to Australia over 16 years ago from Ecuador. Dr Paul holds a Clinical Diploma in Palliative Medicine (Royal Australian College of Physicians) and practices as a Rural General Practitioner in Wangaratta. He is also a medical educator for the RACGP and Director of the Medical Degree Year 3 Rural Pathway – University of Melbourne. Dr Paul was awarded the RACGP 2023 Victoria GP of the Year.

This session will provide a general overview on the palliative care journey of patients accessing general practice, and how digital health platforms can support optimisation of care.

Learning outcomes:

  • Learn strategies to initiate conversation and provide support to patients through their palliative care journey.
  • Describe common challenges faced by palliative care patients and how general practices can manage them.
  • Implement a range of Digital Health tools to optimise palliative patient care
