Western Victoria Primary Care Network (PHN) appointed Everybody’s Business to undertake an area-specific scoping of the aged care service system with the aim to gain an overview of the strengths and issues with the current local aged care system, as well as gather ideas and suggestions about what could support people to age well in place.
